Nursing Home Weekend Admission: What to Ask Before a Friday Transfer

A Friday hospital discharge can create pressure to accept a nursing-home bed quickly.

Weekend admissions are common, but staffing patterns, pharmacy delivery, therapy evaluations, physician coverage, and administrative support may differ from a weekday morning.

The question is not whether a weekend admission is bad. It is whether the receiving facility has everything needed to provide safe care from the moment your parent arrives.

Confirm the facility has accepted the clinical needs

Ask whether admissions reviewed the current:

  • diagnoses;
  • medication list;
  • transfer level;
  • wound orders;
  • oxygen;
  • dialysis schedule;
  • feeding tube;
  • therapy needs; and
  • behavioral or cognitive needs.

A bed reservation is not the same as clinical acceptance.

Confirm medication availability

Ask:

  • Has the pharmacy received the orders?
  • Will routine medications be available for the first evening?
  • What happens if a medication is missing?
  • Is an emergency pharmacy available?
  • Are controlled medications or insulin already arranged?

Friday evening is a poor time to discover the contracted pharmacy needs a clarification from the hospital.

Ask who will complete the admission assessment

Find out which nurse will receive the resident and what assessment occurs on arrival.

Ask whether the nurse will verify:

  • skin;
  • pain;
  • medications;
  • fall risk;
  • transfer method;
  • diet;
  • swallowing precautions;
  • wounds;
  • oxygen; and
  • vital signs.

Ask about physician or practitioner coverage

Determine who handles a new problem after admission.

Ask:

  • Is a physician or advanced practitioner on site?
  • Who is on call?
  • How quickly can new orders be obtained?
  • What happens if the resident’s condition worsens?

Ask when therapy begins

If the move is for rehabilitation, ask:

  • When will PT evaluate?
  • When will OT evaluate?
  • Is speech therapy available if needed?
  • Is weekend therapy scheduled?
  • Does a Friday admission mean waiting until Monday?

The answer can affect the first several days of recovery.

Confirm equipment is ready

If the resident needs a wheelchair, lift, special mattress, oxygen, wound supplies, commode, or other equipment, ask whether it is physically present before arrival.

Bring a family handoff packet

Include a current medication list, allergies, emergency contacts, advance-directive information, baseline function, recent hospital summary, and any care instructions the facility asked you to bring.

A practical example

A resident transfers from the hospital at 6 p.m. Friday.

The SNF has the room, but the family asks one more question about a time-sensitive Parkinson’s medication.

Admissions discovers the pharmacy order has not been activated.

The nurse resolves it before transport.

One question prevents the resident from arriving without a critical medication plan.

Nine questions before a Friday transfer

  1. Has the facility clinically accepted the resident?
  2. Are all medication orders active?
  3. Who performs the arrival assessment?
  4. Who is the weekend medical provider?
  5. When will therapy begin?
  6. Is required equipment in the room?
  7. Is the diet order ready?
  8. Who should family call over the weekend?
  9. What problem would require transfer back to the hospital?
