Medicare Coverage During a Hospice Election: Questions About Unrelated Care
Choosing Medicare hospice changes how care related to the terminal illness and related conditions is covered.
It does not mean Medicare stops covering every other health problem.
Original Medicare can still cover otherwise covered services for health conditions that are unrelated to the terminal illness and related conditions, with the normal deductibles/coinsurance that apply.
Hospice covers the terminal illness and related conditions
Once hospice is elected, the hospice benefit is generally responsible for care related to the terminal illness and related conditions.
The hospice team arranges the covered care under the hospice plan of care.
Families should contact the hospice before seeking related hospital, ambulance, or other services.
Unrelated conditions can still be covered
Medicare’s hospice guidance says Original Medicare still pays for covered benefits for health problems that are not part of the terminal illness and related conditions.
Examples depend on the individual.
Do not decide on your own that a condition is unrelated.
Ask for the hospice determination
Medicare says a beneficiary can request a list of items, services, and drugs the hospice has determined are unrelated to the terminal illness and related conditions, including why.
The hospice must provide that list within the applicable timeframe.
This can help outside clinicians and families understand billing.
Nursing-home room and board is separate
If a person lives in a nursing home and elects hospice, Medicare generally does not cover the nursing-home room and board solely because hospice was elected.
Hospice and the nursing home coordinate different responsibilities.
Emergency/hospital care needs coordination
Medicare warns that hospital outpatient/inpatient or ambulance care related to the terminal illness generally needs to be arranged by the hospice team.
Care unrelated to the terminal illness can follow ordinary Medicare coverage rules.
When uncertain, call hospice first if the situation permits.

Frequently asked questions
Can someone see their regular doctor while on hospice?
Yes; the Medicare hospice benefit has rules about attending professionals and related/unrelated care. Coordinate with hospice.
Does hospice pay nursing-home rent?
Generally not simply because the resident is on hospice.
Who decides whether a service is related?
The hospice makes coverage determinations under the hospice benefit; beneficiaries have rights to information and appeal processes where applicable.
The practical goal
For every outside service during hospice, ask:
Is this related to the terminal illness, who arranged it, which Medicare benefit pays, and what cost sharing applies?
