When a Facility Says Insurance Has Approved Care but the Plan Disagrees

A family may hear:

Facility: “Insurance approved it.”

Plan: “We have no authorization.”

Do not choose which one to believe based on confidence or tone.

Ask for the actual authorization details and reconcile the exact member, facility, service, dates, and status.

First clarify what the facility means by “approved”

It could mean:

  • clinically accepted by the SNF;
  • plan request submitted;
  • plan verbally indicated likely approval;
  • authorization approved;
  • network verified; or
  • payment estimate completed.

Ask:

“Do you have an actual plan authorization number and approved service dates?”

Call the plan with exact identifiers

Have:

  • member name/ID;
  • facility name/address;
  • facility NPI/CCN if available;
  • requested service;
  • requested start date;
  • authorization/reference number from facility; and
  • hospital/referring provider.

Ask the plan to read back the current status.

Check the service dates

An authorization may be valid only for:

  • a specific admission date;
  • a defined initial period;
  • a specific facility;
  • a particular level of care; or
  • a limited number of days before review.

If the transfer is delayed, the authorization may need updating.

Check provider identity

A facility can have multiple billing identifiers or related entities.

Make sure the plan’s authorization matches the actual location/provider that will bill.

Get written confirmation when possible

Request portal/fax/written confirmation of:

  • approved service;
  • provider;
  • dates;
  • authorization number;
  • member cost; and
  • continued-review requirements.

Save it with the facility’s admission confirmation.

Do not transfer solely to “test” the authorization

If coverage is materially unresolved, ask the hospital/facility for the safe interim plan.

Clinical acceptance, bed availability, and insurance authorization remain separate.

Frequently asked questions

Is a fax from the plan enough?

It can be important evidence; confirm it matches the actual member, provider, service, and dates.

What if the plan later denies after authorizing?

Ask for the written reason and review the plan’s appeal/protection rules.

Does the facility have to call the plan?

Facilities often coordinate authorization, but the beneficiary can also call the plan to verify status.

The practical goal

Replace the word “approved” with five fields:

authorization number + service + provider + dates + conditions.

If those do not match between the facility and plan, the issue is not resolved.
