An Explanation of Benefits Says You May Owe Money: Is It a Bill?

An Explanation of Benefits (EOB) is usually a statement from a Medicare Advantage or other health plan describing claims and costs.

It is generally not the provider’s bill.

Before paying a rehab balance, reconcile the EOB with the facility/provider invoice and any later claim adjustments.

Identify the document

Check:

  • plan name;
  • member name;
  • claim number;
  • provider;
  • service dates;
  • claim status; and
  • date the EOB was generated.

A later corrected EOB may replace an earlier one.

Find the member-responsibility amount

An EOB may show:

  • amount billed;
  • plan-allowed amount;
  • plan paid;
  • deductible;
  • copayment;
  • coinsurance;
  • noncovered amount; and
  • amount the member may owe.

The provider’s billed charge can be much higher than the actual member responsibility.

Compare it to the provider bill

If the provider bill is higher, ask why before paying.

Watch for “pending” or reprocessed claims

The plan may:

  • request records;
  • reverse a prior decision;
  • reprocess the claim;
  • apply another payer; or
  • issue a corrected EOB.

Ask whether the claim is final.

Network and authorization issues can appear on the EOB

If the EOB shows an out-of-network or unauthorized service, verify:

  • exact facility/provider;
  • authorization number;
  • approved dates;
  • network status on service date; and
  • whether the provider billed under the expected identifier.

Do not confuse an EOB with a Medicare Summary Notice

Original Medicare beneficiaries receive MSNs.

Medicare Advantage plan members receive plan-specific claim information/EOBs.

The format and appeal process can differ.

Frequently asked questions

If an EOB says “you may owe,” do I pay the plan?

Usually the provider sends the actual bill. Read the plan’s instructions.

What if there is no bill yet?

Wait for the provider invoice unless the plan/provider tells you otherwise, but keep the EOB for reconciliation.

What if the EOB and bill disagree?

Call the plan and provider before paying the disputed difference.

The practical goal

Treat the EOB as a claim explanation, then match it to the actual invoice.

Pay only after the dates, provider, plan decision, and member responsibility line up.
