How to Read a Medicare Summary Notice After a Rehab Stay

A Medicare Summary Notice (MSN) is not a bill.

People with Original Medicare receive MSNs summarizing Part A and Part B claims. After a skilled nursing stay, the Part A MSN can help the family compare what the facility billed, what Medicare approved, and the maximum amount the beneficiary may owe.

Read the claim before paying an unexpected facility balance.

Check the dates first

Match the MSN service dates to:

  • SNF admission;
  • hospital readmission;
  • SNF discharge;
  • benefit-period day count; and
  • any period the resident was not receiving covered SNF care.

If the dates do not match, call the facility/Medicare.

“Amount charged” is not the same as what you owe

The MSN can show:

  • provider/facility billed amount;
  • Medicare-approved amount;
  • amount Medicare paid; and
  • maximum amount you may be billed.

Do not pay the full provider charge merely because it is the largest number on the page.

Check whether the claim was approved

If a line is denied or not covered, read the associated notes.

The note may explain:

  • coverage rule;
  • missing information;
  • benefit exhaustion;
  • service not covered; or
  • another reason.

Compare the MSN to the facility statement

Create a side-by-side review:

ItemMSNFacility bill
Service dates
Medicare paid
Coinsurance/liability
Medigap/other payer
Facility credit
Remaining balance

A facility statement may arrive before all secondary insurance payments are posted.

Review appeal information

The MSN contains information about appeal rights and deadlines for Original Medicare claim decisions.

Do not confuse an MSN claim appeal with a fast appeal of a SNF service-termination notice; those are different processes.

Use the Medicare account for current claim status

Medicare says processed Part A/B claims are often visible in the secure Medicare account before the paper MSN arrives.

Use the official account or 1-800-MEDICARE when reconciling an unclear claim.

Frequently asked questions

Is the MSN a bill?

No.

Should I pay the “amount charged”?

No. Compare the “maximum you may be billed” and actual provider bill/other insurance.

What if the facility bill is higher than the MSN says I may owe?

Call the facility and Medicare/plan to reconcile the difference before paying.

The practical goal

Use the MSN to answer:

What service was billed, did Medicare approve it, what did Medicare pay, and what is the maximum liability shown for the beneficiary?
