What to Ask About Clinician Access in a Nursing Home
A nursing home is not a hospital, but residents must remain under physician care and need a workable way to obtain medical evaluation when conditions change.
Ask how the facility’s clinician model works in daily practice—not only whether a doctor “comes by.”
Know the federal baseline
Federal nursing-home rules require that each resident remain under the care of a physician.
The physician must personally visit at least once every 30 days for the first 90 days after admission and at least once every 60 days thereafter, subject to regulatory provisions. In a skilled nursing facility, certain required visits after the initial physician visit may be alternated with a physician assistant, nurse practitioner, or clinical nurse specialist as allowed by the rule.
Facilities must also provide or arrange physician services 24 hours a day for emergencies.
Those are regulatory requirements—not a description of the facility’s entire clinical model.
Ask who the attending clinician will be
Questions:
- Can the resident keep an existing physician?
- Which clinicians normally attend residents here?
- Is there an on-site medical group?
- How are NP/PA visits used?
- How does family contact the medical provider?
Residents have federal rights regarding choice of attending physician, subject to the physician meeting regulatory and facility requirements.
Ask what happens after hours
“If my parent develops new shortness of breath, fever, confusion, pain, or another change at 11 p.m., what happens before anyone decides about hospital transfer?”
Ask who assesses, who is called, and how medical orders are obtained.
Ask about routine visits beyond minimum intervals
A resident with unstable conditions may need more frequent assessment than the regulatory visit schedule.
Ask how visit frequency changes with need.
Ask about specialists
Clarify:
- facility-provided specialists;
- outside appointments;
- telehealth;
- transport;
- records;
- follow-up orders; and
- who incorporates specialist recommendations into the care plan.

Frequently asked questions
Is a doctor physically in the building 24/7?
Federal rules require physician services to be provided or arranged 24 hours a day for emergencies; that is not the same as an on-site physician at all times.
Can an NP replace every physician visit?
No. The federal rule contains specific requirements and limits.
Does a nursing home have to use my parent’s doctor?
Residents have a right to choose an attending physician if that physician meets the applicable requirements and is willing/able to provide the required services.
The practical goal
Know who provides routine care, who answers after hours, who responds to change, and who closes the loop with specialists and family.
