A Rehab Appeal Was Successful: What Should You Confirm Next?

A successful rehab coverage appeal answers one important question: the reviewer disagreed with the earlier decision for some defined scope.

It does not automatically mean every future rehab day is approved or that every bill has already been corrected.

After winning, confirm the decision’s dates, services, billing effect, and next review point in writing.

Read the decision letter

Identify:

  • appeal level;
  • service involved;
  • dates covered;
  • facility/provider;
  • whether the decision fully or partially favors the beneficiary;
  • whether services should continue; and
  • any conditions or next-review date.

Do not summarize the result only as “we won.”

Ask the facility and plan to update their records

Send or confirm receipt of the appeal decision with the appropriate billing/authorization teams.

Ask:

“What authorization or claim status changes because of this decision, and when will your system reflect it?”

Record the reference number.

Reconcile the bill

If the resident was charged or asked to pay during the disputed period, ask:

  • Which charges will be reversed?
  • Which coinsurance/deductible remains?
  • Has a claim been reprocessed?
  • Will a refund/credit be issued?
  • Does Medigap/secondary insurance need a corrected claim?

Do not assume the account will correct itself.

Confirm ongoing coverage separately

A favorable appeal may cover a specific termination period or decision.

Ask whether:

  • coverage continues through a specific date;
  • another continued-stay review is scheduled;
  • new prior authorization is needed;
  • a new NOMNC can later be issued; or
  • discharge planning still proceeds.

A successful appeal does not promise indefinite coverage.

Keep discharge planning active

If the resident still expects to leave soon, continue work on:

  • home support;
  • equipment;
  • medications;
  • follow-up;
  • transportation; and
  • caregiver training.

Winning coverage time is most useful when the family uses it to strengthen the next transition.

Frequently asked questions

Can the facility issue another coverage-ending notice later?

Potentially, if circumstances or coverage determination change. Read any new notice on its own facts.

Does the successful appeal erase coinsurance?

Not necessarily. Covered services can still have applicable cost sharing.

What if the bill is not corrected?

Use the written decision, claim number, and account statement to ask the plan/provider for reprocessing or correction.

The practical goal

Turn a favorable appeal into operational follow-through:

approved dates → updated authorization → corrected claim/bill → continued-care plan → next decision point.
