Durable Medical Equipment After Rehab: Rental, Ownership, and Coverage Questions

Durable medical equipment used during a rehab stay and equipment needed after discharge are not always billed the same way.

During a Medicare Part A-covered SNF stay, the facility generally provides equipment needed for the covered stay. After discharge home, medically necessary DME may be covered under Part B when the requirements are met.

Before discharge, verify the home DME order and supplier arrangement—not just “equipment ordered.”

Identify the exact item

Write:

  • equipment name;
  • brand/model if relevant;
  • accessories;
  • size;
  • order date;
  • ordering provider; and
  • intended home use.

A wheelchair or hospital bed needs to match the resident and home environment.

Verify the supplier

Medicare requires covered DME to come from a Medicare-enrolled supplier.

Ask whether the supplier:

  • is enrolled in Medicare;
  • participates in Medicare;
  • accepts assignment;
  • serves the home address; and
  • has received the order/documentation.

Rental versus purchase

Medicare covers different DME in different ways.

Depending on the item:

  • rental may be required;
  • purchase may be required;
  • the beneficiary may have a choice; or
  • ownership may transfer after a certain number of rental payments.

Do not assume the equipment becomes the resident’s property immediately.

Ask about beneficiary cost

After the Part B deductible, beneficiaries generally pay 20% of the Medicare-approved amount when the supplier accepts assignment.

Medigap or other insurance may reduce the cost.

Ask for the estimated Medicare-approved/member amount, not just the supplier’s retail price.

Confirm delivery before discharge

Ask:

  • delivery date/time;
  • setup responsibility;
  • training;
  • after-hours contact;
  • repair process; and
  • backup if delivery fails.

Essential equipment should be in place before the resident relies on it at home.

Frequently asked questions

Does the wheelchair used in rehab automatically go home?

No.

Can we buy equipment ourselves and ask Medicare to reimburse later?

Coverage depends on order, supplier enrollment, and other rules. Verify before purchasing.

Does rented DME eventually become owned?

Some equipment can, but rules vary by item.

The practical goal

Treat home equipment as a separate discharge workflow:

order → supplier → Medicare participation → rental/purchase terms → delivery → training → repair/ownership.
