Who Pays for Prescription Medicines During a Skilled Nursing Stay?
Who pays for medicine in a nursing home depends heavily on why the resident is there and which Medicare benefit is paying for the stay.
During a Medicare Part A-covered skilled nursing facility stay, drugs are generally included as part of the covered inpatient SNF services. During long-term nursing-home residence outside a covered Part A stay, Part D or another prescription arrangement may apply.
During a covered Part A SNF stay
Medicare states that drugs received in a SNF are generally covered by Part A as part of the inpatient treatment during a covered stay.
Medicare’s SNF coverage information also lists medications among covered SNF services.
Ask the facility pharmacy/billing office how the medication is included in the SNF payment.
Long-term nursing-home residence is different
For a resident living in a nursing home outside a covered Part A SNF stay, Medicare drug coverage generally works through a long-term-care pharmacy that contracts with the resident’s Part D plan.
The pharmacy is often contracted with or operated in coordination with the facility.
Do not pay a retail-pharmacy bill without checking the coverage period
If a family receives a separate medication bill during what they believe is a covered Part A stay, ask:
- Was the stay actually Part A-covered on that date?
- Was this drug excluded from consolidated billing for some reason?
- Did the family use an outside pharmacy without facility coordination?
- Was the claim billed to the wrong payer?
- Was the resident already in a non-Part-A long-term-care period?
Specialty drugs need early coordination
Some medications may require special sourcing, authorization, or handling.
Tell the SNF pharmacy about specialty drugs before transfer and confirm who will supply the next dose.

Frequently asked questions
Should I bring home medicines into the SNF?
Only if the facility tells you how it will handle them. Do not assume resident-supplied drugs can simply be administered.
Does Part D pay during a covered Part A SNF stay?
Drugs furnished as part of the covered SNF stay are generally handled under Part A, not ordinary retail Part D billing.
What changes after Part A coverage ends?
The resident’s long-term-care pharmacy/Part D or other payment arrangement may become relevant.
The practical goal
Always ask:
What is the resident’s payment status on this date, and which pharmacy/benefit is responsible during that period?
