Leaving Skilled Nursing and Returning Soon After: Coverage Questions to Ask
Leaving a skilled nursing facility does not always close the Medicare coverage story.
If a resident returns soon after going home, the length of the break affects whether a new qualifying hospital stay is needed, whether the same benefit period continues, and how many SNF days remain.
Use exact dates. “Recently” is not precise enough for Medicare coverage questions.
If the break is less than 30 days
Medicare’s SNF booklet explains that when the break in SNF care is fewer than 30 days, a new three-day qualifying inpatient hospital stay may not be required, as long as the person meets the other coverage requirements.
The existing benefit period continues.
That means the resident has only the unused SNF days remaining from that benefit period.
If the break is 30–59 days
Medicare says additional SNF coverage generally requires the person to meet all coverage requirements again.
The prior benefit period has not yet ended because the 60-day break has not been reached.
Ask Medicare or the plan to confirm the exact situation.
If the break reaches 60 days
The old benefit period can end after 60 days without inpatient hospital or SNF skilled care.
If the person later meets the requirements for a new covered stay, a new benefit period can begin with up to 100 SNF days available.
That is not a guarantee the full 100 days will be covered.
A new medical problem may change the clinical picture
The return may follow:
- fall;
- infection;
- hospitalization;
- worsening mobility;
- new wound;
- failed home plan; or
- another condition.
The facility and treating team must evaluate the current need for skilled care.
Medicare Advantage can differ
A Medicare Advantage plan may use network and authorization rules and may waive the traditional three-day inpatient requirement in some circumstances.
Ask the exact plan.

Frequently asked questions
Can someone return to a different SNF?
Potentially, if the new facility accepts the person and coverage requirements are met.
Does returning within 30 days guarantee coverage?
No. The resident must still meet the other Medicare requirements.
Does a 60-day break guarantee a new stay?
No. It may end the old benefit period, but a new covered stay still requires the applicable eligibility criteria.
The practical goal
For a return after discharge, calculate the break length first, then verify the benefit period, remaining days, hospital requirement, current skilled need, facility acceptance, and plan authorization.
