What to Verify When a Facility Has Changed Its Name

A nursing home may rebrand, join a new chain, change its operating name, or change ownership.

A new name can make older reviews and inspection records harder to find.

Match the facility by stable identifiers before deciding whether the history belongs to the same site.

Start with the address and CCN

For Medicare/Medicaid-certified nursing homes, the CMS Certification Number (CCN) is a key identifier.

Record:

Current name: ______

Address: ______

Phone: ______

CCN: ______

Legal business name: ______

CMS Provider Information includes these fields.

A name change is not automatically an ownership change

Check CMS ownership and change-of-ownership data.

Possible situations:

  • same owner, new brand;
  • new management company;
  • chain change;
  • legal ownership change;
  • merger/acquisition;
  • facility reopened under another certification; or
  • simple marketing rebrand.

Do not infer the answer from the sign.

Preserve historical inspections

If the CCN and site continuity indicate the same certified facility, older surveys under the previous name may remain highly relevant.

Search inspection records by CCN when possible.

Check the date of change

Build a timeline:

Old name: ______

New name: ______

Name-change date: ______

Ownership-change date: ______

Administrator change: ______

Most recent survey before change: ______

First survey after change: ______

Ask what actually changed operationally

Ask:

  • administrator?
  • director of nursing?
  • staffing vendor?
  • policies?
  • pharmacy?
  • therapy?
  • medical director?
  • ownership?
  • building?

A rebrand can accompany major operational change—or none.

Frequently asked questions

Does a new name wipe out old citations?

No. Do not assume that. Match certification/site identity.

Does the same address prove it is the same provider?

Not always. Use CCN/legal/ownership records too.

Why do old reviews use a different name?

They may reflect the same physical facility before rebranding. Verify the timeline.

The practical goal

Follow the facility identity, not the logo.

The address, CCN, legal business name, and ownership timeline help connect the current home to its real history.
