How to Review a Nursing Home’s First Bill for Errors
The first nursing-home bill may cover a transition between several payment arrangements.
A resident can arrive under Medicare-covered skilled care, owe coinsurance later, transition to private pay, have Medicaid pending, or receive separately billed services.
Review the dates and payer status before reviewing the dollars.
Build a timeline
Admission date: ______
Medicare/SNF period: ______
Private-pay period: ______
Medicaid pending/approved dates: ______
Hospital/leave dates: ______
Discharge date: ______
A charge can look duplicated when it actually belongs to another payer period.
Match the bill to documents
Compare the admission agreement, written rate quote, Medicare Summary Notice if Original Medicare, plan EOB, Medicaid notice, pharmacy statement, facility ledger, and receipts.
Do not treat the facility bill as the only source of truth.
Check room charges
Ask what room type is billed, whether the rate is daily or monthly, how hospital/leave days were handled, and whether a private-room premium was added.
Under federal rules, certain items/services cannot be separately charged when included in Medicare/Medicaid payment, while requested extras may be treated differently.
Check Medicare cost sharing
Original Medicare SNF cost sharing is based on benefit-period days, not calendar months.
Verify covered days, coinsurance days, denied/noncovered services, and secondary insurance payments against the MSN or plan EOB.
Review extras
Ask for descriptions for charges such as beauty/barber, personal items, private duty aide, transportation, room upgrade, phone/TV, special requested food, pharmacy, equipment, or other ancillary services.
Ask who authorized the charge.
Look for missing credits
Confirm the statement reflects deposits, family payments, insurance payments, refunds, adjustments, and Medicaid payments where applicable.
Send a discrepancy request
For each disputed line:
Date: ______
Charge: ______
Why unclear: ______
Conflicting document: ______
Correction/explanation requested: ______
Avoid saying “the whole bill is wrong” unless you have itemized evidence.
Frequently asked questions
Should I pay an unclear bill while it is reviewed?
Ask about due dates and dispute procedures. Do not ignore the bill, but this article cannot decide whether a specific amount is owed.
Can I compare the bill to Medicare.gov?
Use the MSN or plan EOB for covered claims. Long-term private-pay room charges are different.
What if a charge appears twice?
Check date, provider, description, units, payer adjustment, and whether one line reverses another.
The practical goal
Audit in this order:
dates → payer status → room rate → covered/noncovered services → extras → credits/payments
Then challenge the remaining inconsistent lines.
