When a Nursing Home Administrator Leaves: What Families Should Recheck

The nursing-home administrator is a major operational leader, but an administrator leaving does not automatically mean the facility changed ownership or that care will deteriorate.

CMS Provider Information even includes a measure for the number of administrators who have left the nursing home, reflecting that leadership stability is relevant information.

Use a departure as a trigger to recheck accountability—not as a verdict.

Confirm the leadership plan

Ask:

  • Who is the interim/current administrator?
  • When did they start?
  • Who is the director of nursing?
  • Who handles family concerns?
  • Who has authority after hours?
  • Is a permanent administrator being recruited?

Get current contact information.

Separate leadership change from ownership change

Check CMS ownership data and ask the facility whether:

  • owner changed;
  • chain changed;
  • management company changed; or
  • only the administrator changed.

Do not describe the facility as “sold” unless ownership evidence supports that.

Recheck unfinished improvement work

If the previous administrator was overseeing:

  • plan of correction;
  • staffing initiative;
  • family council issue;
  • renovation;
  • billing correction;
  • grievance;
  • infection-control change; or
  • another promise,

ask who owns it now.

Write down the new responsible person and due date.

Watch staffing and communication

Leadership transitions can affect staff confidence and family communication.

Track:

  • new vacancies;
  • nursing leadership stability;
  • agency use;
  • family meeting schedule;
  • response times; and
  • unanswered issues.

One change does not establish a trend.

Check grievances

Federal resident-rights rules require a grievance process.

Confirm the current grievance official/contact and how pending complaints are being handled.

Frequently asked questions

Is administrator turnover a Care Compare data point?

CMS Provider Information includes administrator turnover information.

Does one departure mean the home is unstable?

Not necessarily. Look at broader turnover, staffing, survey findings, ownership, and current operations.

Should we move a resident because the administrator left?

A departure alone is not enough to make that decision. Reassess actual care and unresolved risks.

The practical goal

When leadership changes, make sure accountability does not disappear with the person who left.

Reassign every open issue, verify current contacts, and distinguish leadership transition from ownership change.
