What to Ask About Charges When Care Is Provided by an Outside Agency
An assisted-living resident may receive services from an outside home-health agency, therapist, hospice, private aide, or clinician.
That can create two sets of bills and two organizations performing related tasks.
Before outside care begins, write down which organization does what and who bills for each part.
Name the outside provider
Record:
Agency/provider: ______
Service: ______
Payer: ______
Schedule: ______
Contact: ______
Do not accept “outside nursing” as a complete description.
Ask the assisted-living community what remains its responsibility
An outside provider does not automatically replace community staff duties.
Ask:
- What care remains included in the assisted-living service plan?
- Does the community reduce any fee?
- Does it charge for coordination?
- Does it charge an escort/access fee?
- Who calls the outside provider for changes?
- Who handles emergencies between visits?
Ask the outside agency what it bills
For Medicare home health, qualifying services follow Medicare home-health eligibility and plan-of-care rules.
Private-duty services may charge hourly minimums.
Hospice has its own benefit structure.
Ask for the provider’s written billing and coverage explanation.
Watch for similar but different services
Example:
Assisted living: medication reminders/assistance
Home health: skilled medication assessment/teaching
Or:
Assisted living: routine personal care
Private aide: one-to-one companionship
These can coexist.
Check minimum hours and cancellation terms
Private agencies may require:
- minimum shift length;
- weekly minimum;
- cancellation notice;
- holiday premium; or
- travel charge.
Add those to the care budget.

Frequently asked questions
Should assisted-living fees drop if home health visits?
Not necessarily. Home health may provide a skilled service that is separate from the community’s housing/personal-care responsibilities.
Can Medicare pay an outside nurse in assisted living?
Medicare home health can sometimes be provided in assisted living when eligibility and other requirements are met.
Is paying two providers always double billing?
No. Compare the actual service and payer.
The practical goal
Before agreeing to outside care, create one responsibility map:
task → provider → payer → family cost → contact
Then the family can see whether charges overlap or simply cover different work.
