Changing Nursing Homes Without Changing States: Medicaid Questions
Moving from one nursing home to another within the same state often does not require starting Medicaid eligibility from scratch.
But Medicaid eligibility alone does not guarantee that the new facility can accept and bill for the resident.
Confirm the destination facility
Ask:
- Is the facility Medicaid-certified?
- Does it have an appropriate Medicaid NF bed/contract arrangement?
- Has admissions clinically accepted the resident?
- Can it meet the current care needs?
- Does managed Medicaid authorization apply?
Ask whether the state/plan needs notice or authorization
Depending on the state and delivery system, the transfer may require:
- eligibility case update;
- managed-care authorization;
- level-of-care update;
- PASRR information;
- new patient-pay routing;
- transportation approval.
Protect the resident’s records
Ensure the receiving facility gets:
- medication/orders;
- care plan;
- diagnoses;
- treatment needs;
- PASRR information;
- advance directives;
- Medicaid/member details; and
- patient-pay information.
Ask about the old facility ledger
Before transfer, reconcile:
- patient liability;
- personal funds;
- refundable charges;
- outstanding optional-service bills; and
- final statement.

Frequently asked questions
Does Medicaid choose the new facility?
No. The resident/family can consider qualified options, but admission and availability matter.
Can the current nursing home block the transfer?
Resident rights and safe transfer requirements apply; disputes may require ombudsman/legal help.
Does the patient-pay amount change?
It can if income/deductions change; otherwise it may remain similar, but use the state’s written determination.
The practical goal
A same-state transfer should coordinate admission + Medicaid billing + records + patient funds, not just transportation.
