When the Nursing Home’s Medicaid Estimate Differs From the State’s Decision

Nursing-home business offices often estimate Medicaid eligibility or the resident’s expected patient-pay amount before the state issues a final decision.

That estimate can be useful.

It is not the official Medicaid determination.

Common reasons for differences

  • updated Social Security/pension amount;
  • spouse allowance;
  • medical-expense deduction;
  • different effective date;
  • income counted differently;
  • state resource decision;
  • facility used an estimate before approval;
  • clerical error.

Start with the state notice

The state Medicaid agency determines eligibility and official post-eligibility liability.

Give the facility a copy of the current notice if appropriate.

Ask the business office to update its ledger.

If the state notice itself seems wrong

Do not ask the facility to “override” it.

Contact Medicaid and use reconsideration/fair-hearing rights if needed.

Frequently asked questions

Should we pay the facility estimate while waiting?

Ask the facility about pending-Medicaid billing terms and keep detailed receipts.

Can a nursing home charge the family personally for the difference?

Admission agreements, Medicaid rules, and guarantor protections matter. Do not sign personal liability casually.

What if the facility says the state is wrong?

Ask the state for the official calculation and appeal process.

The practical goal

Use the nursing home’s number for planning, but use the state’s written Medicaid determination for the official eligibility/patient-pay baseline.
